(Spoilers) She and Zeref kissed and the result of their shared curse is what killed her. Though.. apparently her body is comatose in a lacrima (probably spelled that wrong).

She used one of the most powerful spells there was "Fairy Law" on yuri when he was possessed door an evil power transforming him into a blue skull making mavis use the spell for the first time.

But only resulted in her not being able to age not death.
The way she died is because of Zeref its a long story and it's in one of the chapters of the manga but basically because of the curse that the both have Mavis died
CHAPTER 450 SPOILER My theory is when Zeref and Mavis kissed Zeref understand how valuable a life can be even meer so his curse -ankhseram- did kill her
