It started sometime before the wedding but it was her subconscious telling her she wanted a child...

Really that was how she pictured what renesmee would look like but through the whole book renesmee was thought to be an EJ b4 she was born.

The child was, in a way, Bella's and Edward's son of how Bella first saw Renesmee.

The dream was what led Bella to think that she was at first expecting a baby boy.

In her first dream, she had just been thinking of the immortal children story before she fell asleep and it was playing on her subconscious. She probably wound up giving the kid one of Edward's old traits because that'd endear the boy to her more.

In her seconde dream, her body was trying to tell her that she was pregnant. She was wrongly guessing that she'd have a son.
