Do u think Prim would survive if Katniss had not volunteered for her?

In my opinion, no. She is very skilled in medicine, yes, but the career pack would take her down. Even if she formed an alliance with Rue and/or Peeta, her survival chance would be slim.

She would've died in the bloodbath. She's quite smal and weak.
