I think he did answer the question with "yes, I did" later on and also told the guy how he and Stacy met. So I guess he meant Stacy - would make sense since he used to(!) love her.

No, House did answer coma guy's another question "You ever been in love?" But not this one! That's why I say Cuddy is somebody else.

Nope, House never answered the question. Also, remember how Cuddy asked him, "Do you like me?" in Insenstive? House looked away, and seemed awkward. If Cuddy hadn't continued after that, I'm betting there would've been an awkward silence between them. So, why can't House answer whether he likes her or not? Because he does.
I guess Son of a Coma Guy is just another proof of it.

No, I think he realised that he hadn't really loved anyone else. I think it was one of those moments when he realises the reality of his situation and then changes the subject or deflects in order to avoid his own thoughts and emotions.
