I would have to say there are plenty of other answes I could have added but these were the oens that came to mind. I hate all of these scenarios but getting killed because of a Noob T-mate next to you carrying a plasma on his face pisses me off the most!

Does "T-mate gets stuck and takes you out 2!" really occur that often? Out of 200 or so games, it occurred exactly ONE time... and we were in a pretty tight area; neither of us would've been able to clear out in time.

"A T-mate betrays you for a weapon" OR a flag; now that happened every 20 or 30 games, easy.

ugh. tough decision. next would be the stupid grenade thing. otherwise, my sister(whenever we're teammates) always tries to get a weapon first. if i get it, she finds a different weapon and then kills me.
