I really honestly have no idea. There are so many directions the writers could go in, I'm not even going to attempt to speculate (atleast until more spoilers are released).

It might be job wise;maybe someone gets suspended or demoted.

I might not survive the lows, but it'll be worth it for the highs.

I think because someone will leave - I say either 13 , Foreman or Taub - people might re-think their lives. But, since it's something that will need to be dealt with in the beginning of season 6, I'd say it might be related to some kind of re-structuring of the whole PPTH - hence Chameron coming back into the picture.

im not thinkin its a "hospital" change more than a hospital personal change.

what if, Cuddy is the thing that changes and she leaves PPTH and that's what changes the whole hospital. Cameron steps back in as DoM hence her getting more screen-time, and chase would too if the show focused more on Cameron. now that i'm typing this out and it's not just floating in my head this seems less and less likely because they've already been there, they aren't going to rehash it. but what if cuddy does leave and is replaced by someone entirely new? dear god, why oh why am i thinking about all the possible ways Cuddy could go?!?!
